There are two options for completing this form. Fill out the editable form digitally on a computer (this will not work on a mobile device), or print the form and complete it by hand.

When we receive your completed form, we will connect with you to set up a one-time interview to explain our ministry and hear more from you so we can discern how to serve you best.

If using the print version of the form, please submit it to Keith Stoltzfus, Pastor of Growth and Equipping.

After the initial interview, you will be assigned to a counselor who will arrange a time to meet with you regularly so you can begin the process of growth and change.
